National Lutheran Communities and Services Home Health Social Worker in Winchester, Virginia

A Social Worker provides social work services to clients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Services Director.
Understands and adheres to established social work standards of practice and myPotential at Home policies and procedures Understands and promotes principles of continuous performance improvement Participates in the coordination of myPotential at Home services with the services of others community resources; uses community resources. Maintains contact with community support groups and provides professional expertise as required Observes confidentiality and safeguards all client information Serves as a resource person to team members, clients, physicians and other allied healthcare providers Develops a cooperative relationship and communicates effectively and professionally with physicians Accepts responsibility for regular attendance and punctuality Maintains current knowledge of Medicare Conditions of Participation for social work services Immediately reports any accidents, incident, lost articles or unusual occurrences to Clinical Director Participates in client care conferences Plans, coordinate all social services within myPotential at Home Documents all client/family services provided as required by myPotential at Home policy Assists physician and other team members in understanding the significant social and emotional factors related to health problems Participates in the development of plan of care Prepares clinical and progress notes Participates in discharge planning process Other duties as assigned JOB QUALIFICATIONS
Master’s degree from a school of social work accredited by the Council on Social Work Education and has a minimum of one year experience as social worker in a health care setting required Three to five years’ Social Work and one year experience in home health preferred Acceptance of philosophy and goals of myPotential at Home Ability to exercise initiative and independent judgement Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal Must have and maintain valid driver’s license
