OSF Healthcare Registered Nurse - Palliative Care - Days in United States
The Palliative Nurse provides clinical support, education and development, and case management for OSF St Joseph Medical Center. The Palliative Nurse assists the Palliative Providers in responding to referrals, educating/mentoring nurses and other clinicians in evidence-based palliative care, assisting patients with post hospital discharge care planning, functioning as case manager for patients of St Joseph Medical Center Palliative Care Program. Provides care for patients and family/ significant others across the life span based on physical, psychosocial, educational, safety, and related criteria appropriate to the age of patients served in assigned service area. The Palliative Care Nurse will have 24/7 accountability for the Palliative Care Program.
Minimum of 3-5 years of clinical experience
Hospice/Palliative Care Nursing Certification or to pursue certification within 2 years.
Currently Registered Professional Nurse in the State of Illinois
Excellent communication, organizational, and interpersonal skills
Demonstrated leadership ability and capable of priority setting.
Strong knowledge of the nursing process and problem solving.
Computer experience (word processing and data analysis)
Understanding of the principles of performance improvement, case management, discharge planning, utilization review, and medical records
Understands the basics workings of Joint Commission, DRG’s, and Medicare, Medicaid, and managed care reimbursement. Has a proven record of teamwork.
Ability to work independently.
Baccalaureate degree in nursing is preferred
CHPN Certification preferred
