Either Case Management or home visiting experience.
DUTIES AND RESPONSIBILITIES
This is a professional position working in the Healthy Start Program that covers three counties: Levy, Gilchrist and Dixie County. This position provides care coordination, education and community referrals for pregnant women, infants and children to age three. These services may be provided in the home, clinic or a community setting.This position is headquartered in Levy County and supervised by the Nursing Program Specialist. This position may occasionally provide services in Gilchrist and Dixie counties.
