Works in collaboration with other members to provide services to patients and their families. These services include assistance with determining goals of care, provision of emotional and social support, assistance with decisions about end of life care, and collaboration with community agencies.
Facilitates communication between parents, significant others, physicians, nurses, and community resource personnel.
Promotes and encourages use of existing hospital resources for family/parent support.
Maintains database of patients referred to Palliative Care Services.
Attends Neonatal, Pediatric and Adult Inpatient Unit rounds.
Assumes responsibility in the recruitment and retention of qualified personnel.
Assists in the development of policies, procedures, and standards reflective of current practice/literature.
Participates in orientation of new personnel.
Ensures ongoing communication with patient/family to provide education related to the patient's condition, expected course of treatment, and desired outcome.
Must be able to handle fast paced environment and work independently. Must be able to communicate clearly, verbally, and in writing. Must be able to make sound decisions within limited time elements and to carry out responsibilities in a professional manner. Must be able to work effectively with other professionals. Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
Education, Experience and Certifications
Graduate from an accredited School of Nursing with 3 or more years of experience in a tertiary/quaternary center. Current licensure to practice as a Registered Nurse or temporary license as a Registered Nurse Practitioner in the state of North Carolina. Basic Life Support for Healthcare Provider (BLS HCP) from AHA. Neonatal and/or Pediatric nursing background is essential for Peds Palliative Care Coordinator. Certification in palliative Care Nursing preferred. Certification as an APN preferred.
