If selected as a provider for this clinical service position, you will be required to provide proof of education and transcripts (and proof of licensure, if applicable) and pass all required background checks prior to hire. You will also be required to complete MHP’s credentialing process and attend new hire orientation/training before you will be able to begin work.

Purpose:

The Behavioral Health Home (BHH) Care Coordinator provides the coordination of a client’s care needs and plan in collaboration and continuous partnership with clients and their family/caregiver(s), any specialty providers and staff, and community resources in a team approach.

Job Summary:

The Care Coordinator is largely responsible for constructing, completing and coordinating the care plan with the client and the care team. The Care Coordinator also serves as the contact point, advocate, and informational resource for clients, care team, family/caregiver(s), and community resources

Essential Functions:

Roughly 80% of time is spent on Care Coordination duties, 10% on Health and Wellness Coach duties and 10% on Client Service Advising duties. Please refer to those job descriptions for detailed duty descriptions.

· Welcome new clients to Mental Health Partners and identify care team and care coordinator role to clients as well as provide a brief orientation to the facility and any relevant policies

· Help facilitate client access to appropriate medical and specialty providers inside and outside of MHP

· Address any red flags at inquiry (guardianship, mental health decision making rights) and assist client in completing all required intake forms and signatures

· Assist in obtaining and maintaining current releases of information for all clients involved in BHH

· Communicate with clients regularly who are in need of follow up and work to support them in obtaining their services and problem-solving barriers such as transportation, communication among and between providers etc.

· Assist in the development a care plan and goal setting when a client is not in specialty care emphasizing employment, education and community involvement

· Regularly monitor engagement, effectiveness and client progress with care plans and facilitate changes in treatment as needed.

· Request reassessment from Behavioral Health Professional or other Health Home services to improve treatment effectiveness, outcomes and/or engagement.

· Ensure that the various providers in the client’s care have the information necessary for optimal care and enlist Client Service Advisors to help obtain any forms, letters or records needed and available to the relevant provider(s)

· Connect the client to all other needed health and behavioral health professionals and relevant medical resources to help the client along the care continuum and ensure optimal outcomes for each episode of care. Resources include:

o Specialty behavioral health (including Suboxone Clinic Support)

o Psychiatry and/or Crisis Intervention Services with high risk/high needs clients

o Department of Health and Human Services (DHHS) client care coordination per contract

o Home Health Care Set-up

o Patient Assistance Program (PAP) Coordination and Management

o Primary Care Provider

· Facilitate Behavioral Health Professional re-assessment when needed and consult with supervisor and other team members regarding panel or care team assignments and managing complex cases

· Coordinate transitions of care along the care continuum with the inter-professional team to ensure the goals of a care plan are met and outcomes are improved

· Assist the client in transitions between levels of care or between different types of care at different facilities and assist with the identification of “high-risk” clients and flag in electronic health record. Transitions may include:

o Internal Referral to Specialty Care and Transfer Coordination

o PPC Referrals

o CIS referrals

o Hospital and Discharges - Coordination between Nursing/Medical Care with Hospital DC's in partnership with hospital liaison and Coordination of Nursing/Medical Care with ; Nurse to Nurse SBAR Handoff

o Jail/Probation coordination and discharge first appointments

· Facilitate and attend meetings between client, family/caregiver(s), care team, and community resources as needed

· Review active/inactive status of attendance/participation in care and review regularly with supervisor and care team – complete agency discharges when appropriate and approved by supervisor

· May assist with some case management and health and wellness coach activities when needed

· Conducts supportive counseling with evidence based brief interventions to help with proactive measures to avoid crisis, de-escalation techniques within a crisis and to assist with social, interpersonal and skill development

· Provides support in the management of the milieu and work environment

· Maintains thorough, accurate and timely documentation of the provision of services, assessments, evaluations, and progress toward the completion of client’s treatment goals consistent with clinical and administrative policies and procedures

  • Meets defined individual and department goals, activity metrics and Key Performance Indicators

· Promotes, supports and demonstrates MHP’s mission, vision and values through both behavior and job performance and adheres to policies, procedures, Compliance Plan and Standards of Conduct on a day to day basis

· Effectively interacts and communicates with other MHP staff/clients/customers/partners/etc.

· Regular and consistent attendance is required to perform other essential functions of the job

Education/Experience/License/Certification

· Relevant Bachelor’s degree required; Master’s degree in relevant field preferred

· Minimum 2 years prior experience in a clinical setting required

· Must be 21 years old, have safe driving record, a current Colorado driver’s license and ability to travel within Boulder and Broomfield Counties
