Provides and ensures quality pre-bereavement and bereavement services to terminally ill patients and their families in accordance with the Hospice philosophy, mission statement, policies and procedures and will all laws, rules, regulations and standards applicable to agency operations and teammate's job functions.
Completes pre-bereavement and bereavement assessments.
Provides individual, family and group counseling as needed to patients and families.
Complete the psychosocial portion of the comprehensive assessment on an as needed basis.
Leads grief support groups.
Participates in the coordination of Memorial Service.
Completion and follow-up of suicide risk assessments.
Participates in the Hospice Patient and Family Service Department on-call rotation.
Supports the agencys accreditation and compliance activities.
Primary work setting is patient/family home environment. Potential exposure to pathogens, patientï¿½s blood, body fluids and household chemicals. Frequent movement to and from Hospice Center and patient/family residencies. Some work in climate controlled office setting and facility appropriate for group meetings and in-services. Must meet requirements of the organizationï¿½s pre-employment screening.
Education, Experience and Certifications
Masters Degree in Social Work, Divinity, Counseling or Expressive Therapies from an accredited school required. Minimum of one year experience in bereavement counseling, family system theory or related field required. Strong counseling skills to provide individual, family and group support. Adapt to flexible schedule as necessary. Effective listening and strong communication skills. Must maintain expertise in knowledge of accreditation, regulatory and compliance standards. Function as part of an interdisciplinary team as well as work independently. Proficient computer skills. Reliable transportation; current NC Drivers License; acceptable driving record. Clean National Sex Offender Registry record check.
