Patient Pathways is on a quest to find the newest member of our
Dialysis Coordinator team for Tulsa, OK.
Patient Pathways is a new rapidly-growing team within DaVita, the nation's largest independent provider of kidney dialysis. As an integral part of this team of individuals, you will serve as a liaison between patients, physicians, hospitals, clinics, and healthcare insurance providers. We provide discharge planning for patients. We seek to utilize your knowledge base in patient education including medical insurance by connecting you face-to-face with our patients, and allowing you to deal effectively with their unique and sometimes difficult situations. We are looking for candidates with the following qualifications:
High School Diploma or equivalent is required, Bachelor’s Degree strongly preferred
Strong customer service skills in face-to-face interaction settings
Strong communication skills
Multi-tasking skills and ability to prioritize is a MUST
Strong work ethic and desire to serve patients
Case Management experience is highly preferred
Knowledge of Medical Insurance (Medicare/Medicaid/Commercial) highly preferred but not required
Valid driver’s license and ability to travel locally
Must be travel and work in area hospitals in Tulsa, OK
Must be able to work - Monday –Friday within the hours of 8am – 5pm
32-40hrs a week with full time benefits*
Experience in patient education is highly preferred
Medical experience is highly preferred
Experience with hospital settings is highly preferred but not required
Dialysis experience preferred : not required
