Care Coordinator Req #: 30113945 Category: Nursing Facility: Beaumont Hospital Taylor Department: Care Management Schedule: Casual/Contingent Shift: DaysHours: 8- 4:30 Job Details:GENERAL SUMMARY:Coordinates the care for groups of patients as it relates to disease management, resource utilization, access, discharge planning, quality, service and efficiently across the continuum. This is an interventional role maintaining a patient centered focus.ESSENTIAL DUTIES:1. Responsible for managing case load of patients that include facilitating care during stay, planning and expediting plans for safe and effective discharge.2. Works together with Alternate Site Coordinator with hand offs for alternative placement while maintaining coordination of case.3. Coordinates care by considering all patient needs.4.Uses Interqual criteria and works collaboratively with Utilization Management Coordinators to obtain authorization for care and maximize reimbursement5.Determines and assures appropriate status and level of care.6. Uses defined resources to guide decisions including Medical Director Care Management, Physician Advisors, and Management staff.7. Uses critical thinking and effective judgment skills to determine alternative courses of care.8. Judiciously uses tools designed to expedite care while being cost effective.9.Tools include, cost transfer, medication assistance, cab vouchers etc.10.Initiates notification and communication to patients/families as issues of lack of coverage for services arise. This includes services during the stay and post hospital stay.11. Conveys information completely and timely.12.Effectively interacts with others.13.Delivers excellent customer service by exhibiting the Beaumont Health Standards.14.Provides assistance to individuals in a manner that takes into account their special needs and age-related needs.15. Certification, Licensure, Registration: Current and unrestricted State of Michigan Registered Nurse license, Current American Heart Association BLS for Healthcare Providers certification.Other Qualifications: Excellent interpersonal and communications skills, both verbal and written, Current clinical assessment skills, ability to act in an autonomous, self-directed manner, ability to receive multiple stimuli from multiple sources
