Additional Job Details:Full-Time,Day,40 hours,Weekly, Industrial Rehab Experience or 2 years of out patient orthopedic experience preferred. Will work on-site at job sites such as manufacturing and municipalities providing early intervention and evaluation and treatment of work related or personal injuries. Generally a day job Monday through Friday first shift. Will require an early start on some days at 5:00 am and others at 6:30 am. Must be flexible and able to travel frequently to job sites and between our Ascension Employer Solutions clinics.

What You Will Do

Job Summary:

The Physical Therapist evaluates and conducts medically prescribed physical therapy treatment programs. Will work on-site at job sites such as manufacturing and municipalities providing early intervention and evaluation and treatment of work related or personal injuries Will also include a good deal of preventative care and personal strength and conditioning. Complete functional capacity assessments, ergonomic assessments and develop preventative programs to decrease work related and personal injuries. May include clinic based coverage at our Occupational Health Clinics also for pre-placement screens. Work as part of a that consists of OT's, PT's, LAT's, RN's, NP's and wellness professionals. Must be flexible and willing to travel on a frequent basis. This position will require early starts with some as early as 5:00 am.

Responsibilities:

  • Performs initial and on-going assessments of patient's condition.

  • Establishes, revises and evaluates a plan of care which is appropriate to problems identified and involves the patient/family.

  • Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.

  • Oversees activities of physical therapy assistants, students and other support personnel.

  • Coordinates patient care with other members of the healthcare team to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.

  • Provides education and resources to patient and/or patient's family based on clinical implications.

  • Documents all assessments, plans, interventions, and evaluations including service charges.

Qualifications

Licenses/Certifications/Registration:

  • Required Credential(s):

  • LicensedRehab Therapistspecializing inPhysical Therapy (RPT)credentialed from thestate of Wisconsinobtained prior to hire date or job transfer date.

  • Preferred Credential(s):

  • CertifiedBLS Providercredentialed from theAmerican Heart Association (AHA).

Education:

  • Bachelor's Level Degree

  • Bachelor's Degree or Diploma or Master's Degree or Doctorate in a Physical Therapy Program required

Work Experience:

  • No experience required.

  • 1-2 Years Industrial Rehab prefrered
