Social Worker MSW Req #: 30111222 Category: Clinical/Allied Health Facility: Family Medical Center Department: Family Medicine BMG Troy Schedule: Full time Shift: Days Hours per week: Social Worker MSW Job Details:General Summary:Provides supportive counseling, psychosocial assessment on each patient/family to determine assistance required, design, implementation and documentation of a casework plan. Assists patients and families in recognizing, understanding, accepting and adjusting to substance abuse issues.Primary Responsibilities and Duties:*Complete bio-psycho-social evaluation/intake to establish presence of Substance Use Disorder, determine stage of change, determine frequency of ongoing behavioral therapy, and determine the appropriate treatment setting and level of treatment intensity. Engage patient designated partners in care throughout the recovery process. Complete patient assessment checklist for office-based therapy.*Assesses and evaluates the patient and/or family's social, emotional, cultural, financial and environmental needs and develops a therapeutic plan for intervention with the patient and/or family. Interprets those needs to the attending physician and other pertinent patient care team members.*Provides direct social work service and counseling to patients and their families and/or group of patients and their families in relation to substance use disorder.*Will conduct initial assessment, and provide follow-up behavioral counseling for identified patients with substance use disorder (SUD).• Will connect SUD patients with appropriate resources and work alongside the peer recovery coach to meet FMC patients identified with SUD in the hospital prior to discharge. * Identify and refer patients to a higher level of care for those requiring more intensive treatment as appropriate utilizing ASAM patient placement criteria for the treatment of SUDs.*Provide an integrated approach to treatment planning based on the client's strengths, weaknesses and identified problems and needs.*Provides consultation to the hospital staff, community agencies and other persons seeking guidance to solve problem of patients and either families.*Prepare relevant reports and relevant records in a timely manner consistent with all policies and procedures and following the guidelines required for reimbursement, licensing, accreditation and tracking of clinical treatment.*Participates or assists in department committees, projects, etc., as assigned.* Maintains a current level of knowledge regarding the field by attending required staff meetings, in-services and reading relevant professional journals.Standard Qualifications:Education: Master's degree in counseling, social work or psychologyExperience: Candidates with a minimum of five (5) years work experience in chemical dependency counseling will be given preference.Licensure: Licensure in the state of Michigan in the applicant's discipline.
