POSITION SUMMARY: The Community Health Worker II (CHW) is part of an interdisciplinary care team consisting of physicians, clinical professionals and social workers to assist in the cross continuum approach to hospital transitions. This position will be a key component to achieving collaborative practices that exemplify MultiCare Health System s commitment to patient centered care, community engagement, and focus on transition of care for patients with physical and behavioral healthcare needs. The Level II CHW has significant behavioral health experience and/or is bilingual to be able to serve specific patient populations covered by MultiCare Health System. MINIMUM REQUIREMENTS: Experience in utilization management, community resources, and/or financial and quality issues affecting patient care desirable Community Health Worker experience desirable EDUCATION/LICENSES/CERTIFICATIONS: Required: Minimum of a Bachelor s degree in a healthcare related or behavioral health field, or an equivalent combination of education and experience in a health care or behavioral health setting Additional Requirements: 2 years behavioral health experience, OR bilingual in a population that is Representative of the community we serve OR Master s Degree in Health Related field.
