CLOSE DATE: OPEN UNTIL FILLED ***Applicants must upload a cover letter and resume to their application in order to be considered for this opportunity.*** ****2 POSITIONS AVAILABLE**** TITLE: Mental Health Counselor DEPARTMENT: Puentes REPORTS TO: Clinical Supervisor HOURS: MONDAY-FRIDAY, 9:00AM-5:00PM RANGE: $22.84/Per hour + 5% SUMMARY: The Mental Health Counselor works as a member of the Puentes treatment team to provide culturally and linguistically appropriate mental health services to Latino adults. ESSENTIAL DUTIES AND RESPONSIBILITIES: 1. Participate in culturally specific outreach efforts to engage prospective clients. 2. Conduct screenings and complete necessary enrollment paperwork. 3. Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent; 4. Conduct and document Mental Health Assessments, including mental status exam, DSM diagnosis, and clinical formulation. 5. Develop strengths-based treatment plan with client; Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients; 6. Coordinate case management for individuals in caseload; 7. Conduct educational and therapeutic mental health groups for clients, using approved curriculum; 8. Participate in admission and discharge planning for any client needing hospitalization or sub-acute level of care. Monitor and assure that the health and safety needs of clients are met; 9. Utilize Evidence-based Practices in work with clients in individual and group formats; 10. Coordinate services with all members of client's support network; 11. Maintain client records as required by program standards and funding requirements; 12. Participate in training opportunities to advance skills and knowledge in addictions and mental health; 13. Works within scope of training and experience and seeks consultation when appropriate. 14. Adhere to all state and federal privacy regulations and support compliance with all privacy and security requirements, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. 15. Perform other duties as assigned. MINIMUM QUALIFICATIONS: 1. Master's degree in human services field. At minimum, must meet State guidelines for QMHP. One year of mental health experience preferred. 2. Must have experience working with the Latino culture. 3. Bilingual in English and Spanish required. 4. CADC I certification or ability to obtain within 12 months of hire preferred. Language skill requirement or preference: working with Spanish speaking clients
