CLOSE DATE: Open Until Filled ***Applicants must upload a cover letter and resume to their application in order to be considered for this opportunity.*** TITLE: Housing Case Manager DEPARTMENT: Old Town Recovery Center REPORTS TO: OTRC Program Manager REPRESENTED: Yes SALARY: DOE SCHEDULE: Monday-Friday 8:30am-5:00pm SUMMARY: The Housing Case Manager works as a member of a multi-disciplinary behavioral health care team providing integrated behavioral health services to clients seen at Old Town Recovery Center. The Housing Case Manager is responsible for providing housing support the OTRC mental health teams, ensuring care that is accountable, comprehensive, proactive, well-coordinated and patient-centered. The programs at OTRC address all treatment needs, including crisis management, substance abuse treatment, psychiatric evaluation and treatment, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing. The programs are responsible for proactively managing the healthcare needs of the clients on their assigned team, including medical, mental health, addiction, and health behavior needs. MINIMUM QUALIFICATIONS: 1. Bachelor's degree in Behavioral Sciences field or a combination of at least three year's work, education, training or experience in chemical dependency and/or mental health treatment; 2. Has the competencies necessary to: a. Communicate effectively; b. Understand addictions and mental health treatment and service terminology and to apply the concepts; c. Provide psychosocial skills development; and d. Implement interventions prescribed on a treatment plan. 3. Experience with severe and persistent mental illness (SPMI) preferred
