Intake Coordinator

A local State agency is looking for experienced, professional and reliable candidates for their Intake/Onboarding Coordinator- This person should be highly organized, have experience with customer service, office or administration work, be able to maintain confidentiality and produce quality results in a timely manner.- Experience in a medical office or administration is preferred.- Schedule is Monday - Friday, 8:00am - 5:00pm, located in Midtown Phoenix with a Pay Rate of $16.50/hr.- This is a 3 month assignment that will be continuously renewed based on work performance.

Responsibilities:

JOB SUMMARY

The Intake Coordinator provides the initial assessment and coordination of integrated healthcare services for CMDP enrolled members. This position provides administrative support to the medical management team; serves as the single point of contact for tracking and communicating member specific data and performs ongoing medical case management.

Working hours: 8:00 AM - 5:00 PM

Major duties and responsibilities include:

  • Communication with caregivers and DCS to assist with obtaining timely, appropriate medical and behavioral health services. Enter and maintain assessments, authorizations and pertinent clinical information into the QNXT medical management system

  • Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff to meet the complete healthcare needs of CMDP members

  • Ensure completion of required care coordination activities and document all activities within member medical records

  • Research missing or unclear information contained in member medical records

  • Prepare and disseminate member medical records/reports to providers and clinical staff

  • Coordinate quality transitions to other department programs by identifying and reporting potential gaps or barriers in member treatment plans

  • Provide education and information on available CMDP services

Qualifications:

MAJOR RESPONSIBILITIES/ESSENTIAL FUNCTIONS

Conduct initial review and dissemination of Enrollment Transition Information (ETI) forms received from other AHCCCS plans; secure accuracy of the data; research missing or unclear information; correctly record appropriate information into the QNXT database; communicate with AHCCCS representatives and staff from other health plans if necessary; communicate routinely with CMDP management to discuss and identify possible barriers in the ETI process.

  • Experience in a medical office or administration is preferred.

  • Candidates MUST be able to clear a 7 year background check, obtain a Fingerprint Clearance Card, and sign a non-disclosure agreement.
