ETI CARE COORDINATOR

A local State agency is looking for experienced, professional and reliable candidates for their ETI Care Coordinator role.- This person should be highly organized, have experience with customer service, office or administration work, be able to maintain confidentiality and produce quality results in a timely manner.- Experience in a medical office or administration is preferred.- Schedule is Monday - Friday, 8:00am - 5:00pm, located in Midtown Phoenix with a Pay Rate of $16.50/hr.- This is a 3 month assignment that will be continuously renewed based on work performance.

Responsibilities:

The Care Coordinator receives and processes the Enrollment Transition Information (ETI) forms, which are received from a relinquishing AHCCCS Health Plan when a member terminates eligibility with that plan. The coordinator also completes and forwards an ETI form to the appropriate health plan when a member leaves CMDP (Comprehensive Medical & Dental Program) coverage. All ETI forms are tracked by the Care Coordinator in a timely manner. They identify critical/pertinent health information, enter the member's health information into QNXT database system, and coordinate care needs with the appropriate nursing staff. When further action is needed, the care coordinator completes a request for medical records and forwards the form to the member's primary care physician or specialist. After receipt of medical records, the care coordinator will further assist the nurse to ensure that members receive all appropriate services. This position also participates on the Medical Services phone queue.

Qualifications:

  • Experience in a medical office or administration is preferred.

  • Candidates MUST be able to clear a 7 year background check, obtain a Fingerprint Clearance Card, and sign a non-disclosure agreement.
