Providence Health & Services Physical Therapist-Pelvic Health & Ortho in Newberg, Oregon

Description:

Providence is calling a Physical Therapist (1.0 FTE, Days) to Providence Newberg Medical Plaza, in Newberg, OR.

We are seeking a Physical Therapist who will manage and provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities, taking into account the individual patient special physical or age-related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual functional level and potential.

In this position you will have the following responsibilities:

  • Provides appropriate, timely and effective evaluation and re-evaluation of patients:

  • Incorporates patient and family as appropriate in goal setting.

  • Evaluates patient/client current physical status and develops individualized, measurable and functional treatment goals and establishes interventions as appropriate to patient age, condition, diagnosis, response to treatment and learning needs.

  • Incorporates patient and family as appropriate in goal setting.

  • Re-evaluates and updates goals in a timely manner.

  • Plans and implements a comprehensive and individualized physical therapy treatment plan:

  • Modifies and progresses the program in response to changing patient/client needs.

  • Instructs patient/client and caregivers to assist in achieving treatment goals.

  • Terminates treatment as appropriate in relation to patient/client’s age, condition, diagnosis and functional performance / outcome expectations and discharge criteria.

  • Communicates frequently and effectively in the professional environment with patients, caregivers and others:

  • Provides and maintains accurate, thorough, legible and timely patient related documentation, handwritten and/or computerized, in accordance with department policy and procedure.

  • Communicates frequently and effectively with others involved in patient/client’s care regarding scheduling, treatment plans, progress and discharge planning.

  • Provides non-patient related documentation accurately and in a timely manner to Manager /Lead.

  • Maintains positive working relationships.

  • Demonstrates professional behavior and pursues professional development:

  • Maintains and enhances personal expertise through appropriate professional educational experiences.

  • Maintains required hospital and department competency in environment of care, age-related care, CPR and other required system trainings.

  • Maintains a high standard of professional accountability with respect to appearance, demeanor, safety, ethical and legal issues.

  • Provides a comfortable, orderly, clean, and safe work environment.

  • Keeps informed of health care issues, changing department procedures, and other department information by attending staff/team meetings and/or reading staff meeting minutes, e-mails and newsletters.Enhances personal professional growth by establishing individual goals, providing in-services to others, participating in community activities, and contributing to program development.

Qualifications:

Required qualifications for this position include:

  • Bachelor's Degree from an accredited Physical Therapy Program.

  • Master's Degree from an accredited Physical Therapy Program.

  • Doctor of Physical Therapy (D.P.T.) from an accredited Physical Therapy Program.

  • Occasionally a position will require specialized physical therapy training to meet a particular department need.

  • Current Physical Therapy licensure through the State of Oregon, or temporary license if new graduate awaiting licensing exam results

  • American Heart Association Basic Life Support (AHA BLS) with automated external defibrillator (AED) for Healthcare Professionals, required within 30 days of hire.

  • National Provider Identifier (NPI).
