Northwest Regional Council Case Manager **Revised Minimum Qualifications** in Mount Vernon, Washington
The Northwest Regional Council , as an Area Agency on Aging, funds and/or provides community-based programs to help elders, people with disabilities, and people living with behavioral health disorders live in their own homes and communities for as long as possible, postponing or eliminating the need for institutional care. The Case Manager assists persons who are older and/or disabled in NWRC's Whatcom or Skagit County service areas. The position assesses client needs, develops and implements service plans, and oversees the authorizations of payment to care providers. NWRC is currently seeking to fill 2 FTE Case Manager positions, one each to be sited in the Bellingham and Mount Vernon offices.
