The Care Management Registered Nurse is responsible for ongoing case management coordination through the continuum of care. This individual supports managing complex patients by implementing treatment plans and facilitating patient’s access to appropriate health care services, community resources, in addition to supporting patient self-management and increasing knowledge of his/her disease process. The Registered Nurse (RN) manages therapy for patients enrolled in Care Management, which includes completing assessments, planning, dosing of medications according to physician protocols, and evaluating the patient’s response to treatment. The Nurse supports the physician’s practice by following physician protocol to manage patient’s therapy, related education, triage and coordination of care and referrals when appropriate. In addition to working with the patients and their families, the Nurse interacts regularly with physicians and personnel from within the Marshfield Clinic system and outside to include, but not limited to: Pharmacies, clinic/office/hospital personnel, laboratories and home care nurses to coordinate care of patients.Marshfield Clinic Health System is committed to enriching the lives of others through accessible, affordable compassionate healthcare. Education (For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.)
Required Education:Graduate of an accredited school of nursing.
Minimum Experience Preferred:Five years recent clinical experience in acute or ambulatory care. Experience with patient interaction on the telephone. Involvement with unit-based quality management.
The following licensure(s), certification(s), registration(s), etc., are required for this position, which must be maintained in good standing:
Minimum Required:Current Wisconsin Registered Nurse licensure. Care Management RNs working in the Anticoagulation Service are required to obtain Anticoagulation Management Certification within six months of hire.
