Parkview North Hospital Case Management Specialist-ED in Fort Wayne, Indiana

Parkview Regional Medical Center-Case Management Nursing - Part Time Benefit Eligible, Days, 730a-8p Posted 2019-08-23 Req # 143155 Summary: The emergency department case management specialist collaborates with physicians and other members of the health care team to ensure that patients are placed in the appropriate level of care utilizing Interqual software, Executive Health Resources and other tools as needed. Assists with patient flow through the ED assuring that the appropriate admission status has been assigned. Supports the reduction of 30 day readmissions through collaborative efforts with the ED physicians and PPG offices. # Education: Must hold a BS or BSN degree. If Bachelors degree is in nursing must have a BSN from accredited program. Acceptable accrediting agencies are Commission on Collegiate Education in Nursing (CCNE) Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA). MSN preferred. # Licensure/Certification: Must be an RN with current licensure. Certification in Case Management highly desirable. # Experience: Preferred recent experience in acute care setting. Preferred experience in Case Management activities. Preferred experience or knowledge of InterQual criteria. Demonstrates knowledge of DRGs managed care initiatives payment methods InterQual Criteria and statistical analysis. Demonstrates understanding of managed care concepts. Demonstrates ability to engage in complex clinical decision-making. Demonstrates a strong commitment to interdisciplinary collaboration and communication. Demonstrates critical thinking and analysis skills and conflict resolution skills. Must have good verbal and written communication skills. Must have excellent people skills and the ability to problem solve efficiently and effectively. Must be available to work flexible hours assuming on-call duties on weekends and other times when deemed necessary. Must be available to respond within a reasonable length of time.
