We have an immediate Opening for an Outreach Case Manager Position in our Adult Services Department. This position will report directly to the Outreach Coordinator.

Key Responsibilities

  • This position is a new addition to our Center and is an exciting opportunity for an individual in the mental health field.This position will manage a caseload of client that are deemed “low utilizer” of the services here at Seacoast Mental Health.

  • Utilize their skills in Motivation interviewing, to assist in re-engagement, caring for outreach to clients and/or support system for clients who are engaging in prescribed services.

  • The ideal candidate should be able to accurately describe targeted case management services to clients, Families and community providers. Develop goals and objectives that are consistent with the TCM assessment and provide ongoing monitoring of the clients and/or family’s progress.

Education Requirements:

  • Bachelor’s Degree in human services, social work, counseling or related field to work with adults 18 and over in the home, community and agency.

  • Experience working with systems, an understanding of mental illness, knowledge of community and state resources and an ability to work well within a treatment team required.

  • Strong organizational, liaison, and advocacy skills required. Prior experience in a mental health or human service agency a plus.

  • The Case Manager must hold a valid driver’s license and be able to demonstrate a clean driving and criminal record.

  • CPI, CPR and First Aid training must be completed within the first six months of hire and maintained throughout the duration of employment.
