UnitedHealth Group Field Based RN Case Manager - Fairfield County, CT in Danbury, Connecticut

This position requires ability to visit facilities about half your time and ability to work remotely the other half of time

Field - based RN Case Managers must be able to accommodate up to 50% daily field - based travel within Fairfield County, CT and must be able to accommodate flexible schedule including some evenings and weekends

Primary Responsibilities:

  • Manage targeted population daily census

  • Interact in a collegial and collaborative fashion with hospital clinical staff to include: Nursing staff, Social Workers, UR / UM, physicians, and non-clinical support staff

  • Communicate regularly with Nurse Practitioners/Physician Assistants

  • Review charts for coding quality

  • Review on call logs against patient charts

  • Perform in services at Long Term Care Facilities

  • Promote cost effective health care with aligned health system networks

Required Qualifications:

  • Current, unrestricted RN license

  • 3+ years of clinical experience as a Registered Nurse in a hospital setting, acute care or direct care experience

  • Intermediate level of experience with Microsoft Word, Outlook, Excel, IPhone and IPhone Apps with the ability to type and navigate a Windows environment

  • Reliable transportation and the ability to accommodate daily field - based travel up to 50% of the time within the service delivery area

Preferred Qualifications:

  • BSN

  • Previous experience working in a Managed Care Organization (MCO)

  • Case Management, utilization review or other related experience

  • Certified Case Manager (CCM)

  • Experience working in a field-based role (i.e. home health)
