The Community Hospice has a rewarding opportunity for per diem RNs to work as a part of our Admissions Team in hospice home care.
Evaluates patients for eligibility and interest in hospice services, describes the support and care available from along with the benefits of hospice care. Coordinates with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services. Completes the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team including social workers, chaplains, volunteers, complementary therapists, home health aides and more! Ensures a smooth handoff to the hospice RN Care Manager.

Minimum Requirements :
Valid NYS RN license
Associate’s degree in nursing
1+ year of recent acute care experience
Preferred Requirements :
1+ year of experience in home care
Ability to maintain the highest level of customer service while completing day to day duties and responsibilities
Ability to communicate effectively in a fast pace work environment
Ability to engage within a team setting in the same building as well as members out in the field
Passion to learn, grow and develop in the nursing field
