Horizon is looking for a dedicated Clinical Care Specialist in our outpatient clinics in Erie County!
This is a new entry level clinical position! Under the supervision of the clinic’s Practice Manager, the Clinical Care Specialist will work with a multi-disciplinary team providing clinical support to the counseling staff. This may involve contact with patients and family members, outreach to referral sources and acting as a liaison to community resources.
Duties and Responsibilities
Care coordination telephonic, documentation and educational support – triage patient phone calls; collateral outreach; coordination with outside providers, including completion of referral releases, paperwork, applications and status update reports; medication related calls/refills and education to patient/counselor regarding side effects
Secure insurance authorizations as required for medications, alternate care levels, etc.
Conduct toxicology tests with patients
Ensure follow-up on receipt of physical, medically related information
Respond to walk-ins presenting at the clinic in crisis situations
Review and flag patient records to ensure required updates are prepped and/or made timely
Successfully complete internal training and credentialing process to perform approved clinical services such as educational and process group activities
Ability to relate to mentally ill or chemically dependent individuals and their families of diverse ethnic, cultural, and socioeconomic backgrounds and to interact with them in a positive, supportive and cooperative manner
Strong customer service and interpersonal skills
Good written and verbal communication skills
Good organizational, time management and problem-solving skills
Ability to prioritize work tasks
Ability to complete work accurately and on time despite frequent interruptions

Experience working in a computerized office environment with word processing, database and spreadsheet skills sufficient to prepare correspondence, reports and forms, including the willingness to learn new software applications

Location and Hours
Outpatient program on 3020 Bailey Ave in Buffalo, NY. Shift / Hours: Monday – Friday, some evenings required (Full time)
