The Brookdale Hospital & Medical Center TEMPORARY LICENSED PRACTICAL NURSE (FT/EVENINGS 3:00PM-11:00PM) in Brooklyn, New York
Administers direct nursing care to a group of patients under the supervision and direction of a Registered Nurse and assists in the delivery of patient care within the framework of the nursing process. Collaborates with other members of the health care team in meeting the patient(s) identified needs assigned to him/her during a given period of time. The Licensed Practical Nurse is responsible to render service to acutely ill adult patients ranging from 18 years old and older.
Utilizes observational skills to patient’s total needs. Gather data concerning the patient’s physical status through inspection and conversation.
Assists in collecting pertinent information for completing admission assessment.
Identifies common recurrent patient problems, symptoms, and behavioral changes in relationships and interactions with others
Assists RN with discharge, transfer, and orientation of new patients on the unit.
Provides input to the health-care team in establishing an individualized plan of care for each patient.
Makes sure equipment is available and functioning by testing as per policy.
Updates initial nursing plan for care as patient condition/needs changes.
Administers medication and documents patients’ response to medication.
Utilizes clinical skills in the provision of nursing care delivery.
Assists the health-care team whenever necessary.
Maintains standard of care and demonstrates competency in performing all procedures within prescribed standards.
Provides treatment and care delivery as assigned and within scope of practice standards
Provides health teaching and counseling to patients and significant other.
Guides and assists unlicensed nursing personnel in tasks appropriate to their role.
Observes and reports changes in patient’s condition to the RN and/or physician (physical, emotional and behavioral changes).
Initiates an Occurrence Report for any incidence/situation deviating from the norm.
Completes assignment based upon standards of care.
Documents according to hospital policy.
Successful completion of an accredited Licensed Practical Nurse Program.
New York State Practical Nurse License.
One (1) year LPN experience preferred.
Ability to communicate effectively in English both verbally and in writing
Involves standing, walking, sitting, talking, hearing and bending.
May include some repetitive motions.
