Brockton Hospital Clinical Social Worker, Managed Care/ACO in Brockton, Massachusetts

The clinical social worker, as part of the Complex Care team, engages in a collaborative process with emergency department staff, hospital staff and primary care providers to assess, plan, coordinate and implement services to meet a patient's needs. The social worker assists in the identification of appropriate behavioral health care and other community resouces ensuring that available resources are being used in a timely and cost-effective manner in order to obtain improved patient outcomes and an improved patient expereince. The qualified candidate will have experience in a medical setting as well as expertise in evidence-baed practices relevant to working with patients and thier families with co-occurring mental health and substance use disorders. The social worker will demonstrate cultural competence and understanding of social determinants of health in care delivery. Familiarity with Patient Centered Medical Home and Medicaid Population is a plus. Master of Social Work degree from an accredited graduate program. 3-5 years in a medical setting required, acute care experience preferred. LICSW preferred. LMHC or LCSW may be considered based on experience.
