Upper Chesapeake Medical Center Social Worker in Bel Air, Maryland

Provides social work and discharge planning services for all inpatients accessing care through the UM Upper Chesapeake Health. This position will complete psychosocial assessments, assist with treatment planning, monitoring patient progress, facilitating patient and family meetings, and coordinating and implementing discharge services. Screens patients to identify anticipated needs, interacts with patients and families, so that a safe and timely care plan is achieved. Coordinates and implements discharge and post-acute services for inpatient caseload.

Education & Training: MSW from a school of Social Work accredited by the Council of Social Work Education required. Maryland licensure at the LGSW level.

Work Orientation & Experience: Acute care social work experience preferred. Knowledge of Case Management, Critical Pathways, and Utilization Management preferred. The applicant may require up to three months of detailed job orientation and training to gain specific capabilities to perform essential job functions.

Skills & Abilities: Demonstrate skill in: a) social work casework; b) assessing patient and family dynamics; c) developing and implementing discharge plans; d) brokering services for patient/family with vendors; e) effective critical thinking; f) written and verbal communications; g) prioritize work assignments and manage time effectively to complete duties; and h) age appropriate interpersonal interactions (patients may range from newborn to geriatric adult). Demonstrate knowledge of: a) Medicare and Medicaid regulations and documentation and b) community resources and c) case management principles. Demonstrate ability to a) communicate and collaborate effectively with both internal and external customers (colleagues, Medical Staff, liaisons, and patient/family); b) assess, adapt, and calmly respond to changing and/or crisis environment; c) make independent decisions consistent with current policies, procedures, health care and ethical standards; and d) be available to adapt hours to meet the needs of all customers.
