Seeking a full-time physician trained in either Med/Peds or Family Medicine for an academic practice focused on transitioning young adults with special needs from their pediatric oriented care to adult care. Clinical time in this position will be split between Children's Hospital Colorado (.50 FTE) and a special needs transition program at a nearby Federal Qualified Health Center (.50 FTE). Teaching responsibilities for students and residents are an anticipated part of this position as is participation in other academic activities of the programs at the University of Colorado School of Medicine.
The physician will provide care for patients at a nearby FQHC ambulatory practice or on contract through GIM for a total of 20 hours per week (.50 FTE), with opportunities for precepting residents and students. Faculty share responsibilities for night, weekend, and vacation coverage with other members of the group practice and will participate in decisions regarding the practice operation.
The clinician-educator will have opportunity for academic promotion judged on criteria of demonstrated excellence as a clinician/educator/scholar. Clinician-educators are encouraged to collaborate with other faculty in clinical research projects.
_What is the Adult Special Care Clinic (ASCC)?_
ASCC is a primary care medical home for patients >18yo with highly complex medical care needs.
Who would be eligible for this clinic?Patients >18yo transitioned to ASCC from Children’s Hospital Colorado Special Care Clinic (SCC) or community primary care settings. Clinical criteria for eligibility would include: high paperwork needs, complex medication regimens, chronic ventilation (tracheostomy or non-invasive), tube feeds (gastric, jejunal), parenteral nutrition.
Where would the clinic be housed?This clinic would be housed within the SCC at CHCO, located on the 1stfloor of the outpatient pavilion. The SCC is a well-supported primary care medical home for children with complex needs and the ASCC would gain the efficiency of existing support structures for the SCC.
Where would patients in the ASCC receive subspecialty, inpatient, and Emergency Care?Subspecialty care would be transitioned at 18yo to adult subspecialists in the CU Medicine practice and inpatient/emergency care would take place at University of Colorado Hospital.
Who constitutes the ASCC clinical team?
Two MD/DO providers, each at 0.5 cFTE, will compromise the physician team. A full-time Advanced Practice Provider would support patient care needs as well as full-time Registered Nurse, 0.5 FTE RN Case Manager, and 0.5 FTE Medical Assistant. Registered Dietitian, Clinical Pharmacist, Psychologist, and Respiratory Therapist support would also be available as these clinical services are available to patients in SCC.
How often would the Adult complex primary care clinic be held?Scheduled outpatient physician consultative and follow-up appointments for a half-day Monday-Friday. Urgent clinic visits would also be available throughout the week M-Fr. Each MD provider will be expected to hold, on average, 2.5 clinic sessions per week.
Where would labs/radiology be performed?Routine labs/radiology studies as ordered by primary care could be obtained at CHCO. Labs/radiology tests ordered by subspecialists would be performed at UCH. Results would available across EPIC platforms through Care Everywhere.
Nature of Work:
This is a clinical position, involving outpatient clinical work and care coordination with outpatient subspecialists and inpatient care teams for complex adult patients. The provider may have a primary faculty appointment within the Department of Medicine, Division of General Internal Medicine or Department of Family Medicine with an additional appointment in the Department of Pediatrics. The provider will maintain hospital privileges at Children’s Hospital Colorado and University of Colorado Hospital with electronic medical record access at both institutions.
Examples of Work Performed:
Providing primary care to medically complex adult patients in the outpatient setting. This includes physically seeing patients in clinic in the ASCC and the FQHC sites as well as communication with the clinical teams (APP, RN, etc) to ensure urgent patient needs are met in between clinic sessions. 0.5 FTE in the ASCC consists of 5 half day sessions every two weeks with associated follow up care and consultation. 0.5 in the FQHC will consist of 4 half days per week of clinic.
Participation in nighttime/weekend call rotation to manage ASCC patient/family phone calls outside of business hours.
Coordinating care with a diverse care team that includes adult subspecialists and community providers.
Engaging with inpatient care teams at University of Colorado Hospital to ensure smooth inpatient-outpatient transitions of care. This may include visiting the patient in the hospital or telephone/electronic communication/coordination with inpatient care team.
Precepting residents (potentially pediatrics, medicine/pediatrics, family medicine) with special interest in caring for medically complex patients.
