Instructor - APP Navigator

APP Navigator is responsible for coordinating the care of patients and to serve as a resource for the consulting departments within Oncofertility program. This program focuses on fertility preservation in pediatric, adolescent & young adult patients with medical illness or treatments that threaten their gonadal function, including oncology, hematology and renal patients receiving chemotherapeutic agents, and those managed with bone marrow transplant. The navigator will serve as a clinical resource in this area of specialty throughout the continuum of care from pre-treatment counseling through post-treatment follow-up. The navigator will endorse consistent communication throughout the Oncofertility program and will foster interdisciplinary teamwork.

PRIMARY DUTIES AND RESPONSIBILITIES:

Program Coordination / Management

  • Attend weekly meetings for oncology services as directed and ensure all new patients are reviewed and assessed for potential fertility preservation consult in a timely manner.

  • Coordinate care with cancer center and external care providers and research partners regarding preservation and interventions.

  • Assist with quality improvement measures for the Oncofertility program

o Assist research team with clinical portions of maintaining QI/research databases with onco-fertility patient and treatment information

o Monitor data routinely to present outcomes based measures to the Oncofertility program

  • Assist in the continued development of the Oncofertility program and improving process flow

Patient Navigation

  • Acts as liaison and team leader by facilitating communication and coordination of care between patients, families, health team members and institutions; explains routines and procedures to patients and coordinates patient care information to appropriate sources.

  • Triage each patient’s fertility preservation needs, coordinate pre-consult evaluation and participate in fertility consults in inpatient & outpatient settings and/or via telephone.

  • Assure patient understanding of system processes and provide a single point of contact and guidance for fertility and gynecologic care.

Clinical Roles

  • Provides direct patient care, including assessment, planning, implementation, and evaluation of patients; completes health history and physical examination on patients; conveys findings to physician.

  • Coordinates patient care, orders diagnostic tests, conveys findings to physician, attends inpatient rounds, attends outpatient clinics, coordinates patient appointments, and assists with referrals to outside sources.

  • Accessible for Oncofertility communication and referrals.

  • Clinical role of the program is inclusive of pediatric male and female fertility preservation in addition to the gynecologic late effects of cancer survivors.

Required Qualifications

  • Graduate of an accredited NP/PA program

  • Current Colorado NP/PA licensure; Basic Life Support certification (Online BLS certifications, those without a skills assessment component are not sufficient to meet the BLS requirements.).

Preferred Qualifications

  • 3 to 5 years of experience

  • Previous women's health and adult oncology nursing experience desired but not required
