We are looking for Certified Homes Health Aides (CHHA) to fill a number of positions, including full time, part time and per diem positions to provide care to our patients.
As a member of the interdisciplinary team, the home health aide provides personal care services to terminally ill patients at their residence and in accordance with the plan of treatment as assigned by the RN Case Manager. The role of the CHHA is to increase the level of comfort, maintain personal hygiene and a safe, dignified and healthy environment for the patient and to also provide for temporary respite and relief from total care responsibilities for the primary caregiver. Must be able to utilize computers and be comfortable with EMR.
A high school graduate or equivalent.
Current CHHA Certification within the State. Certification in Hospice and Palliative Home Health Aide desired.
Ability to read, write and follow directions.
Valid driver’s license, and auto insurance, reliable/well maintained transportation.
Bilingual in English/Spanish preferred.
Able to utilize computers and be comfortable with EMR.
