UPMC PA or CRNP, Staff/Senior – Palliative Care/Family Medicine (Altoona, PA) in Altoona, Pennsylvania

UPMC Altoona is hiring a full-time Physician Assistant or a Certified Registered Nurse Practitioner, Staff or Senior level to help support the Altoona Family Physicians and Palliative department located in Altoona, PA. This is a robust Palliative Care service that continues to grow with currently 4 rotating Physicians, 2 Advance Practice Providers (APP), a Social Worker, and a Medical Assistant. The team sees consults within the hospital and three local nursing homes and an outpatient palliative clinic. The APP in this position will rotate both palliative clinic and family medicine practices that are part of Altoona Family Physicians. The APP in this position will work an 8-hour daylight shift Monday through Friday and one weekend about every 7 weeks. Home on-call with weekend rounding is required. A background in palliative care and family or internal medicine is a plus.
The Staff Advanced Practice Provider (APP) is a Physician Assistant and a member of the care delivery team. The Staff APP is responsible for setting the standards for the level and quality of care. The Staff APP has responsibility, authority and accountability for the provision of care through supervision by and collaboration with a physician. The Staff APP manages and provides patient care activities for a group of patients and their families through the application of independent judgment, communication and collaboration with all team members. The role of the Staff APP encompasses the beginnings of leadership, partnership, collaboration and supervision. The Staff APP establishes and maintains collaborative relationships with physicians, other health care providers, patients and their families, to achieve desired patient outcomes throughout the continuum of care. The Staff APP begins to demonstrate a commitment to the community and to their health care profession.
The Senior Professional Advanced Practice Provider (APP) is a Physician Assistant and a member of the care delivery team. The Senior Professional APP is responsible to set the standards for the level and quality of care. The Senior Professional APP has responsibility and accountability for the provision of care through supervision by and collaboration with a physician. The Senior Professional APP manages and provides patient care activities for a group of patients and their families through the application of independent judgment, communication and collaboration with all team members. The role of the Senior Professional APP encompasses leadership, partnership, collaboration and supervision. The Senior Professional APP establishes and maintains collaborative relationships with physicians, other health care providers, patients and their families, to achieve desired patient outcomes throughout the continuum of care. The Senior Professional APP demonstrates a commitment to the community and to their healthcare profession.

Timely and accurate medical record documentation
The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the patients served as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.
The successful completion of anapproved accute care nurse practitioner program is required
BSN, MSN is preferred
Professional nursing experienceand/or nurse practitioner experience is preferred
A graduate of a physicianassistant program accredited by the Accreditation Review Commission onEducation for the physician assistant (ARC-PA) is required
BS or MS candidate is preferred
Experience as a physicianassistant and/or experience in a clinical setting is preferred
PA, Senior Level:
Minimum 3 years of experienceor Completion of a post-graduate residency
The successful completion of anapproved Nurse Practitioner program or a graduate of a physician assistantprogram accredited by the Accreditation Review Commission on Education forthe physician assistant (ARC-PA) program is required.
BSN, MSN preferred for CRNP, BSor MS preferred for PA
Completion of NationalCertification
Must achieve aSolid/Strong/Good performance rating or higher on annual performanceevaluation for consideration and to maintain status annually.
Completion of an OAPP approvedprofessional contribution within one year of hire or promotion into theposition.
Must have department chairapproval for promotion.
Demonstration of clinicalprecepting of one APP student per calendar year for at least two years orevidence of ongoing education of APP students or demonstrate a meaningfulcontribution to student education

Licensure, Certifications, and Clearances:
Must be eligible for licensure. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
